Requirements
to the procedure for issuing an accounting card

Hazardous Inventory Card production facility in the state register of hazardous production facilities - this is a document of the established form, an integral appendix to the certificate of registration of hazardous production facilities, filled out for each hazardous production facility, containing information about the name, signs of danger and type of hazardous production facility, data from the organization’s constituent documents, etc. .

When drawing up an accounting card in paragraphs 5.1, the free right field is filled in.

In the column “Full name of the facility”, enter its full name assigned to the facility when identifying hazardous production facilities of the organization in accordance with the List of typical types of hazardous production facilities for registration in the state register.

Columns 5.2.1 and 5.2.2. are filled in completely if the facility is operated by a branch of the organization. Signed by the head of the branch of the organization who has the right to sign and seal. If there is no branch, only column 5.2.1 is filled in. In the right field of the items, and the “V” sign, the required danger signs, one of the types of object and one or more licensed types of activities are indicated.

Clauses 1.2 and 1.3 of the accounting card respectively provide the address and code of the subject Russian Federation or administrative-territorial entity on the territory of which a hazardous production facility is located.

The points must indicate identified signs of danger, both one sign of danger (if the facility has no other signs) and several signs, for example, if there are both hazardous substances and equipment operating under overpressure, lifting mechanisms. The presence of a hazard sign at a facility is determined by what is listed in Appendix 1 to the Federal Law “On Industrial Safety of Hazardous Production Facilities.”

Determining the type of hazardous production facility establishes the degree potential danger dangerous production facility and determines the amount of the minimum amount of liability insurance for causing harm to life, health or property of other persons and the environment natural environment in the event of an accident at a hazardous production facility.

The items on the accounting map indicate the types of activities for which licenses are required.

Species licensing activities in the facility registration card in the state register of hazardous production facilities are established by Rostechnadzor in accordance with the legislation of the Russian Federation on licensing.

When filling out an accounting card in case of registration of hazardous production facilities by operating applicants who have such an organizational legal form How self employed or an individual entrepreneur without forming a legal entity, some fields of the hazardous production facility registration card, where statistics codes are indicated, may not be filled in due to the absence of such codes from the applicant.

General information about the hazardous production facility registration card and legal requirements for its registration.

Hazardous production facilities are enterprises that include sites, workshops and individual areas. They are legally classified as hazardous production facilities, which is regulated by the use of hazardous substances or work on them. Such substances and work can cause significant harm to the environment and the health and life of employees or the population living in the area where the facility operates.

Legal relations for such facilities are regulated by Law No. 116-FZ of July 21, 1997 “On the safety of hazardous production facilities.” The same law establishes requirements for the registration of hazardous production facilities by objects. Previously, the main document regulating the development of an accounting card, its form and the requirements for its completion and registration was the Administrative Regulations of Rostechnadzor, approved by Order No. 606 of September 4, 2007. Afterwards, the temporary procedure for registration of public health organizations was approved on March 19, 2013 N 31-rp, where the form of the registration card was presented in Appendix 1. At the moment, this form is used when preparing documentation for public health organizations.

Actually, a hazardous production facility accounting card is a document, the form of which is approved by law. It is part of the package of documents that mandatory are necessary for enterprises to register or re-register HPOs with government agencies. At the same time, registration of public benefit organizations is also a legal requirement. Its absence may result in large fines from regulatory organizations. Also, in the absence of a registration certificate, the enterprise will require an industrial safety examination.

Accounting cards are filled out based on the previously carried out identification procedure for hazardous production facilities.

Who can draw up and approve an accounting map - cost and timing.

Filling out the accounting card can be carried out either by the facility’s own employees or by employees of a specialized commercial organization providing this service on a paid contract basis. In the second option, registration of an accounting card takes place within five days, depending on the speed of provision of the necessary data about the object by its management. The submitted data is certified by the seal of the organization and the signature of its head, and receives registration number in the approving body (with the obligatory indication of the name of the organization), certified with an official seal. Do not forget that according to Article 2 of Law No. 116-FZ, responsibility for the data provided for approval of the accounting card rests entirely with the head of the operating organization.

Documents required for approval of the accounting card.

After registration of the accounting card, the procedure for its approval by the controlling authority follows. government organization. To do this, the organization operating the hazardous production facility provides a certain package of documents, which includes:

The information contains information about the organization itself, brief description assigned class, as well as applied technical devices and documentation data regulating its use;
• information about the organization of a legal nature, as well as details of the statutory documents (certificates of registration and registration with tax authority, statistics code, etc.).
• data that provides justification for the safety of the facility;
• an order from an organization to appoint a person responsible for carrying out the identification procedure;
• information about the structure of the organization - the presence of divisions and separate branches in it, as well as data about the objects included in its structure;
• general plans of buildings and structures of the industrial site, as well as their explication;
• list of hazardous substances used in production processes;
• information about previously obtained licenses and permits for the use of technical devices;
• data on the technologies used;
• title documents;
• compulsory insurance contract.

Features of coordination.

An application for approval of a hazardous production facility can be submitted both from the operating organization itself and from its structural unit in case its location differs from the location management organization. In the second case, a note on additional approval by the approving authority at the location is also required dangerous object.

If the object is subject to supervision by other government agencies, it must also receive a mark of approval from them.

The agreed registration card is provided in two copies as part of the package of documents required for registering a hazardous production facility with the state.

Stages of forming an accounting card.

In general, the work on preparing the registration certificate for a hazardous production facility can be divided into the following main stages:

1. Temporary procedure for maintaining the state register of hazardous production facilities

1. This Temporary Procedure for maintaining the state register of hazardous production facilities (hereinafter referred to as the Temporary Procedure) was developed on the basis of Federal Law of July 21, 1997 N 116-FZ “On Industrial Safety of Hazardous Production Facilities” (hereinafter referred to as Law N 116-FZ), resolution Government of the Russian Federation dated November 24, 1998 N 1371 “On registration of facilities in the state register of hazardous production facilities” in order for the territorial bodies of the Federal Service for Environmental, Technological and Nuclear Supervision (hereinafter referred to as the registering authority) to re-register hazardous production facilities with the assignment of the appropriate hazard class on the basis of paragraph 1 of Article 10 of the Federal Law of March 4, 2013 N 22-FZ “On Amendments to the Federal Law “On Industrial Safety of Hazardous Production Facilities”, separate legislative acts of the Russian Federation and on the recognition of subparagraph 114 of paragraph 1 of Article 333_33 of Part Two of the Tax Code of the Russian Federation as invalid" (hereinafter referred to as Law No. 22-FZ).

2. Registration of hazardous production facilities in the state register of hazardous production facilities, re-registration of hazardous production facilities in the state register of hazardous production facilities, making changes to the state register of hazardous production facilities, including exclusion of hazardous production facilities from the state register of hazardous production facilities (hereinafter referred to as registration ) is carried out by the registration authority in accordance with the requirements of Law No. 22-FZ.

3. The registration body carries out registration of hazardous production facilities, with the exception of hazardous production facilities operated by legal entities subordinate to the federal executive authorities and State Corporation By atomic energy"Rosatom", at the location of the operating organization (according to the statutory documents) within a period not exceeding 20 working days from the date of registration of the application documents.

4. The basis for registration of hazardous production facilities in the state register of hazardous production facilities is an application for registration of hazardous production facilities, with the following documents attached:

registration card of a hazardous production facility establishing the hazard class (in two copies for each facility), drawn up in accordance with Appendix No. 1 of the Temporary Procedure, agreed (if necessary) in the manner established by the Administrative Regulations of the Federal Service for Environmental, Technological and Nuclear Supervision for execution state function for registering hazardous production facilities and maintaining the state register of hazardous production facilities, approved by order of the Federal Service for Environmental, Technological and Nuclear Supervision dated September 4, 2007 N 606 (hereinafter referred to as the Administrative Regulations);

The operating organization has the right to submit documents in person or through postal item with a description of the contents and a notification of delivery.

5. Composition additional information and exhaustive cases of their provision are established by the Administrative Regulations.

6. The registration authority monitors the validity of assigning the appropriate hazard class when registering hazardous production facilities in the state register of hazardous production facilities.

7. In order to obtain information and documents necessary for registration of hazardous production facilities at the disposal of the bodies providing government services, bodies providing municipal services, other government agencies, organs local government, in accordance with regulatory legal acts of the Russian Federation, regulatory legal acts of the constituent entities of the Russian Federation, in the event that they are not submitted on their own initiative by the applicant - the operating organization, as well as to verify the information provided by the applicant, territorial body Rostechnadzor independently requests such documents through a system of interdepartmental electronic interaction.

8. When registering hazardous production facilities in the state register of hazardous production facilities, a certificate of state registration hazardous production facilities, issued in accordance with Appendix No. 2 to the Temporary Procedure.

9. When re-registering a hazardous production facility, the registration number previously assigned to the hazardous production facility is retained, unless the registration authority changes (in case of a change, a new number is assigned).

The columns of section 6.2.2 of the accounting card are filled in in case of approval of the accounting card of a hazardous production facility, if the address of the place of operation of the registered hazardous production facility does not coincide with the location of the organization operating it (in accordance with its constituent documents) or the approval of another federal executive body is required, also carrying out supervisory activities at such an object.

When registering hazardous production facilities, information about which is classified as a state secret, the fields in the registration card may not be filled in, indicating the addresses of the organization operating it and the place of its operation.

The registration card for a hazardous production facility is drawn up on one sheet with the back.

Hazardous production facility registration card- this is a document that is a mandatory component of the certificate of registration of a hazardous production facility in the Unified State Register of HPFs (USROPO).

IMPORTANT. Since 2017, the presence of a PPO registration card when registering a PPO is no longer required due to the entry into force of the new administrative regulations approved By Order of Rostekhnadzor dated November 25, 2016 No. 494. In short, the HIF accounting card was combined with information characterizing the HIF.

A hazardous production facility registration card is drawn up based on the results of identifying a hazardous production facility based on quantitative and qualitative characteristics. Filling out HIF accounting cards a labor-intensive process that requires experience and knowledge of legislation, which is best left to an expert organization that specializes in this issue and has experience in this market. After filling out the HIF registration cards, they are required approval by Rostekhnadzor.

By Order of Rostechnadzor dated November 25, 2016 No. 494, a new administrative regulations on registration of hazardous production facilities in the State Regional Public Institution, which combined the registration card of hazardous production facilities with information characterizing the hazardous production facility. A separate development of a hazardous production facility accounting card is no longer required.

HIF accounting cards contain the following information:

• information about the name of the educational institution;
• information about the danger signs of hazardous production facilities;
• information about the type of hazardous production facility;
• information about the legal entity.

NOTE:“the head of an organization operating hazardous production facilities is responsible for the completeness and accuracy of the information submitted for registration in the Unified State Register of Industrial Organizations, in accordance with the legislation of the Russian Federation” ( Federal Law No. 116, article 2, paragraph 5).

A hazardous production facility registration card must be filled out for all hazardous production facilities. The procedure for approving a hazardous production facility registration card is carried out in territorial office(management) of Rostechnadzor, on whose territory a hazardous production facility is located. The HPF accounting map is drawn up in accordance with the current regulatory legal act:

"The registration card for a hazardous production facility is filled out in accordance with the sample design given in Appendix No. 6, on one sheet with the back."(Order No. 606 of 09/04/2007, clause 24.2).
