1. General provisions

1.1. In accordance with the Federal Law “On Education in the Russian Federation” dated January 1, 2001, additional educational programs are being implemented on additional education (Part 2 of Article 12). Additional educational programs include additional general education programs - additional general developmental programs, additional pre-professional programs (Part 4 of Article 12).

1.2. This procedure has been developed in accordance with Part 5 of Article 55 of the Federal Law “On Education in the Russian Federation”.

1.3. This procedure regulates the admission of citizens to study in additional general education programs at the municipal budgetary educational institution "Ust-Tsilma Secondary School" (hereinafter referred to as the Institution)

2. Organization of admission to training

2.1. Admission to additional general education programs for minor students is carried out upon the application of their parents (legal representatives), other citizens - upon their personal application.

2.2. Any person is allowed to master additional general education programs without presenting requirements for the level of education, unless otherwise stipulated by the specifics of the educational program being implemented.


2.3. Admission to study in additional general education programs is issued by order of the director of the Institution within 7 working days after receiving the documents.

2.4. In the case of admission to study in additional general education programs at the expense of individuals and (or) legal entities, the issuance of an order on the admission of a person to study is preceded by the conclusion of an agreement.

2.5. The contract for the provision of paid educational services specifies the information provided for by the Rules for the provision of paid educational services, approved by a decree of the Government of the Russian Federation.

2.6. When admitting to sports, tourism, choreographic (dance) associations of interest, a medical report on the student’s health status is required.

2.7. Admission to additional general education programs is carried out on the principles of equal admission conditions for all applicants.

2.8. The institution has the right to admit citizens for training in additional general education programs beyond the municipal assignment established by the founder of the institution for the provision of municipal services for a fee on the same conditions for the provision of the same services.

2.9. The institution is obliged to familiarize the applicant and (or) his parents (legal representatives) with its charter, with a license to carry out educational activities, with a certificate of state accreditation, with educational programs and other documents regulating the organization and implementation of educational activities, the rights and responsibilities of students. The fact of such familiarization is recorded in the application for admission and certified by the personal signature of the applicant and (or) parents (legal representatives) of the child. When admission is conducted on a competitive basis, the applicant is also provided with information about the ongoing competition and its results.

3. Organization of educational activities for additional general education programs

3.1. The organization and implementation of educational activities for additional general education programs is carried out in accordance with the Procedure for the organization and implementation of educational activities for additional general education programs, approved by order of the Ministry of Education and Science of Russia dated January 1, 2001 No. 000 (registered with the Ministry of Justice of the Russian Federation on November 27, 2013, reg. No. 000).

4. Final provisions

4.1. This Procedure is posted for review on the School’s website.

4.2. The validity period of the Order is unlimited. The provision is valid until a new one is adopted.

The Procedure for admission to higher education programs - bachelor's, specialist's and master's degrees has been approved.

The rules for admission (including admission procedures) to a specific organization are established by it independently to the extent not regulated by education legislation.

Persons with secondary general education are allowed to study bachelor's or specialist's programs. In this case, a document on secondary general education, a document on secondary vocational education or a document on higher education and qualifications is submitted.

Admission is carried out on the basis of the results of the Unified State Examination and (or) the results of entrance tests conducted by the organization independently (in established cases).

For full-time and part-time forms, the start date for accepting documents is no later than June 20. The deadline for accepting documents from applicants based on the results of additional entrance tests of a creative and (or) professional orientation is no earlier than July 7, from applicants based on the results of other entrance tests conducted by the organization independently - no earlier than July 10.

The deadline for completing entrance examinations conducted by the organization independently, and completing the acceptance of documents from applicants without passing the specified entrance examinations is July 26.

Persons with higher education of any level are allowed to study master's programs. In this case, a document on higher education and qualifications is submitted.

Admission is carried out based on the results of entrance tests, the form and list of which are determined by the organization.

For full-time and part-time forms, the start date for accepting documents and the completion date for entrance examinations are determined by the admission rules independently approved by the organization. The deadline for accepting documents is no earlier than August 10

In all cases, when applying for distance learning, the deadlines are set by the admission rules independently approved by the organization.

The procedure applies starting from the 2016/17 academic year.

1. General provisions

1.1. These Rules for admission to preschool education programs (hereinafter referred to as the Rules) have been developed in accordance with:

Federal Law of December 29, 2012 No. 273-FZ “On Education in the Russian Federation”;

Federal Law No. 115-FZ of July 25, 2002 “On the legal status of foreign citizens in the Russian Federation”;

Law of the Russian Federation dated February 19, 1993 No. 4530-1 “On forced migrants”;

2. Organization of reception

2.1. The educational organization accepts children aged _______ to _______.

2.3. Admission of children with disabilities is carried out for training according to adapted programs with the consent of parents (legal representatives) based on the recommendations of the psychological, medical and pedagogical commission.

2.4. Admission of children to the educational institution is carried out during the calendar year, subject to availability of places.

2.5. Before the start of reception, the PA appoints a person responsible for receiving documents and approves the schedule for receiving applications and documents.

3. Enrollment procedure

3.1. Admission of children to educational institutions for training in preschool education programs, as well as in the group (groups) for care and supervision without the implementation of a preschool education program is carried out in the direction of ______________________________________________________,

3.2. To enroll in a PA, parents (legal representatives) of children living in the territory assigned to the PA additionally provide:

3.3. To enroll in a PA, parents (legal representatives) of children who do not live in the territory assigned to the PA additionally provide:

original birth certificate of the child.

3.4. To enroll in a PA, parents (legal representatives) of children who are not citizens of the Russian Federation additionally provide:

a document confirming the applicant’s relationship or the legality of the representation of the child’s rights;

a document confirming the applicant’s right to stay in the Russian Federation (visa (in case of arrival in the Russian Federation in a manner requiring a visa) and (or) a migration card with a stamp of entry into the Russian Federation (except for citizens of the Republic of Belarus), a residence permit or permission to temporary residence in the Russian Federation, other documents provided for by federal law or an international treaty of the Russian Federation).

Foreign citizens and stateless persons submit all documents in Russian or together with a duly notarized translation into Russian.

3.5. To enroll children from families of refugees or internally displaced persons into a public organization, parents (legal representatives) provide:

a forced migrant certificate with information about family members under 18 years of age, or

refugee certificate with information about family members under 18 years of age.

3.6. Parents (legal representatives) of children entering the PA for the first time provide an appropriate medical report.

3.7. The person responsible for receiving documents, when accepting an application, is obliged to familiarize himself with the applicant’s identity document in order to establish the fact of family relations and the powers of the legal representative.

3.8. When accepting an application, the official responsible for accepting documents acquaints parents (legal representatives) with the charter of the public organization, the license for the right to carry out educational activities, educational programs implemented by the public organization, local regulations regulating the organization and implementation of educational activities, the rights and responsibilities of students, as well as these Rules.

3.9. The fact that the parents (legal representatives) of the child have familiarized themselves with the documents specified in clause 3.8 is recorded in the application for admission and certified by the personal signature of the parents (legal representatives) of the child.

The signature of the child’s parents (legal representatives) also records consent to the processing of their personal data and the child’s personal data in the manner established by the legislation of the Russian Federation.

3.10. The person responsible for receiving documents registers the submitted applications and documents in the journal for accepting applications for admission to the PA, about which the parents (legal representatives) of the children are given a receipt for receipt of documents containing information about the registration number of the application for admission of the child to the PA and the list of submitted documents. documents. The receipt is certified by the signature of the person responsible for receiving documents and the seal of the PA.

3.11. The application can be submitted by the parent (legal representative) in the form of an electronic document using public information and telecommunication networks.

3.12. When applying for training in educational programs of preschool education or at the expense of individuals and (or) legal entities, the issuance of an order for admission to a public organization is preceded by the conclusion of a training agreement.

3.13. When admitted to a group (groups) for supervision and care without implementing an educational program for preschool education, the issuance of an order for admission to an educational institution is preceded by the conclusion of an agreement for the provision of supervision and care services in the manner prescribed by the Rules for the provision of paid educational services in an educational institution.

3.14. Enrollment of a child in a public organization is formalized by order of the head of the public organization within three working days after the conclusion of the contract. An order for enrollment in a public organization is posted on the information stand of the public organization and on the official website of the public organization on the Internet within three days after publication.

3.15. For each child enrolled in an educational institution, a personal file is created in which all documents received upon admission are stored.

4. Organization of admission to training in additional general development programs

4.1. Everyone is accepted for training in additional general developmental programs, regardless of place of residence, according to the age categories provided for by the relevant training programs.

The number of places for training in additional general development programs is determined by the educational institution annually, no later than ____________________________.

4.2. Admission to training in additional general developmental programs is carried out without entrance examinations, without presenting requirements for the level of education.

Admission to study in additional general development programs may be refused only if there are no available places. Admission to study in additional general developmental programs in the field of physical education and sports may be denied if there are medical contraindications to specific types of activity.

4.3. Applications for training in additional general development programs are accepted from _________________ to _________________.

4.4. Information about the areas of study for additional general developmental programs, the number of places, the schedule for accepting applications, the rules for admission to training for additional general developmental programs is posted on the information stand of the public organization and on the official website of the public organization on the Internet no later than 30 days before the start of accepting documents.

4.6. To enroll in additional general developmental programs, parents (legal representatives) of minor citizens, along with the application, provide the original birth certificate or a document confirming the applicant’s relationship.

Parents (legal representatives) of minors who are not citizens of the Russian Federation, parents (legal representatives) of minors from families of refugees or forced migrants additionally provide the documents provided for in Section. 3 of these Rules.

4.7. To enroll in additional general developmental programs in the field of physical education and sports, parents (legal representatives) of minor citizens must additionally submita certificate from a medical institution stating that there are no medical contraindications to engaging in a specific sport specified in the application.

4.8. Reception of applications, familiarization of applicants with title documents and documents regulating the learning process, registration of applications, enrollment is carried out in the manner prescribed by Section. 3 of these Rules.

4.9. Enrollment in an educational institution for training in additional general development programs at the expense of the corresponding budget of the budget system of the Russian Federation is formalized by order of the head of the educational institution in accordance with the procedure for organizing the provision of additional education to children, provided for by ________________________________________________.

4.10. 4.11. Orders for admission to training in additional general development programs are posted on the information stand of the public organization and the official website of the public organization on the Internet three days after publication.

Coordination with these bodies is carried out in accordance with the procedure for the adoption of local regulations, determined by the charter of the educational organization.

The procedure for approval of a local regulatory act (by an official or administrative act) is determined in accordance with the instructions for office management of an educational organization.

For admission to the LLC: parents (legal representatives) of children living in the assigned territory, in order to enroll the child in the first grade, additionally present the original birth certificate of the child or a document confirming the applicant’s relationship, a certificate of registration of the child at the place of residence or at the place of stay in the assigned territory territory or a document containing information about the registration of the child at the place of residence or place of stay in the assigned territory; parents (legal representatives) of children not residing in the assigned territory additionally present the child’s birth certificate. Parents (legal representatives) of children who are foreign citizens or stateless persons additionally present a document confirming the applicant’s relationship (or the legality of the representation of the child’s rights) and a document confirming the applicant’s right to stay in the Russian Federation. Foreign citizens and stateless persons submit all documents in Russian or together with a duly certified translation into Russian. Copies of the documents presented upon admission are stored in the LLC for the duration of the child’s education. 10. Parents (legal representatives) of children have the right to submit other documents at their discretion. 11. Upon admission to LLCD to obtain secondary general education, a certificate of basic general education of the established form is presented. 12. The requirement to provide other documents as a basis for admitting children to an LLC is not allowed. 13. The fact that the parents (legal representatives) of the child are familiar with the license to carry out educational activities, the certificate of state accreditation of the LLC, the charter of the LLC is recorded in the application for admission and certified by the personal signature of the parents (legal representatives) of the child. The signature of the child’s parents (legal representatives) also records consent to the processing of their personal data and the child’s personal data in the manner established by the legislation of the Russian Federation 7 . 14. Acceptance of applications for the first class of LLC for citizens living in the assigned territory begins no later than February 1 and ends no later than June 30 of the current year. Enrollment in LLCOD is formalized by an administrative act of LLCOD within 7 working days after receiving the documents. For children who do not live in the assigned territory, acceptance of applications for first grade begins on July 1 of the current year until the empty seats are filled, but no later than September 5 of the current year. LLCs that have completed admission to the first grade of all children living in the assigned territory admit children who do not live in the assigned territory earlier than July 1. 15. For the convenience of parents (legal representatives) of children, LLCs establish a schedule for accepting documents depending on the registration address at the place of residence (stay). 16. When admitting children who do not live in the assigned territory to vacant places, children of citizens who have the right to priority provision of a place in an LLC in accordance with the legislation of the Russian Federation and regulatory legal acts of the constituent entities of the Russian Federation have priority rights. 17. Children with disabilities are admitted to study in an adapted basic general education program only with the consent of their parents (legal representatives) and on the basis of recommendations of the psychological, medical and pedagogical commission. 8 18. Documents submitted by parents (legal representatives) of children are registered in the application log. After registering the application, the parents (legal representatives) of the children are given a receipt for receipt of documents containing information about the registration number of the application for admission of the child to the LLC, about the list of documents submitted. The receipt is certified by the signature of the LLC official responsible for receiving documents and the seal of the LLC. 19. The administrative acts of the LLC on the admission of children to study are posted on the information stand of the LLC on the day of their publication. 20.   1. General provisions

1. This regulation on the procedure for admitting pupils to study in educational programs of preschool education (hereinafter referred to as the Procedure) at the municipal budgetary preschool educational institution "Kindergarten No. 115 of the general developmental type of the city of Vladivostok" (hereinafter referred to as the Institution) was developed in order to comply with the rights of citizens to free public preschool education, improving the quality and accessibility of municipal services: the procedure and grounds for admitting students (hereinafter - Pupil). The procedure directly regulates the provision of municipal services to students (hereinafter referred to as Pupil).

1.2. The procedure is developed in accordance with:

1) the Constitution of the Russian Federation;

2) Federal Law of December 29, 2012 N 273-FZ “On Education in the Russian Federation”;

3) Order of the Ministry of Education and Science of the Russian Federation No. 293 dated 04/08/2014;

4) Order of the Ministry of Education and Science of the Russian Federation dated August 30, 2013 No. 1014 “On approval of the Procedure for organizing and implementing educational activities in basic general education programs - educational programs of preschool education”;

5) Sanitary and Epidemiological Rules SanPiN 2.4.1. 3049-13;

6) Charter of the Institution;

7) other legal acts in the field of education.

1.3. This Procedure was developed in order to ensure the realization of the rights of citizens to publicly accessible, free preschool education, meet the needs of citizens for educational services for preschool children, and provide assistance to families in raising children.

  1. The procedure for admitting Pupils to an educational institution

2.1. Admission of Pupils to study in educational programs of preschool education is carried out in accordance with the rules of admission to study at an educational institution.

2.2. The rules for admission to an educational Institution must ensure the admission to the Institution of all Pupils who have the right to receive preschool education and who live in the territory to which the Institution is assigned.

2.3. Admission of Pupils to an educational Institution is carried out by the head of the Educational Institution in accordance with the legislative and regulatory acts of the Russian Federation. Admission of pupils to the Institution is of an application nature. The basis for admission to the Institution is the application of the parents (legal representatives) of the student for admission to the Institution.

2.4. Admission of Pupils to the Institution is carried out on the basis of:

1) a referral voucher issued by the department for work with municipal educational institutions of the city of Vladivostok;

2) applications from parents (legal representatives) for admission of the Pupil to an educational institution (Appendix No. 1);

3) an identity document of one of the parents (legal representative) of the Pupil (original and photocopy);

4) a copy of the Pupil’s birth certificate;

5) details of a card or passbook for transferring compensation for part of the parental fee for maintaining the Pupil in the Institution;

6) medical card about the Pupil’s health status;

7) a certificate of registration of the Pupil at the place of residence or at the place of stay in the assigned territory or a document containing information about the registration of the Pupil at the place of residence or at the place of stay.

2.5. The application for admission by parents (legal representatives) of the Pupil shall indicate the following information about the child:

1) last name, first name, patronymic;

2) date of birth;

3) last name, first name, patronymic of the pupil’s parents (legal representatives), place of work (position), contact telephone number;

4) residential address of the Pupil and his parents (legal representatives);

The fact that the parents (legal representatives) of the child are familiar, including through public information systems, with the license for the legal conduct of educational activities, the Charter of the preschool institution is recorded in the application for admission to the preschool institution and is certified by the personal signature of the parents (legal representatives) of the child.

When admitting a Pupil to the Institution, the head of the Institution is obliged to familiarize parents (legal representatives) with the Charter, the license to carry out educational activities, educational program documentation, and other documents regulating the organization of the educational process.

2.6. The application for admission and the documents attached to it, submitted by the parents (legal representatives) of the Pupil, are registered by the head of the Institution in the register of applications of the parents (legal representatives) of the pupil for the admission of the Pupil to the Institution.

2.7. After registering the application, the parents (legal representatives) of the children are given a receipt for receipt of documents containing information about the registration number of the application for admission of the child to the educational institution and the list of submitted documents. The receipt is certified by the signature of the official of the educational institution responsible for receiving documents, and the seal of the educational institution (Appendix No. 2).

3.For admission to an educational institution:

3.1. Parents (legal representatives) of children living in the assigned territory, in order to enroll the child in the Institution, additionally present the original birth certificate of the child or a document confirming the relationship of the applicant (or the legality of the representation of the child’s rights), a certificate of registration of the child at the place of residence or at the place of stay at assigned territory or a document containing information about the registration of the child at the place of residence or place of stay;

3.2. Parents (legal representatives) of children who do not live in the assigned territory additionally present the child’s birth certificate.

3.3. Parents (legal representatives) of children who are foreign citizens or stateless persons additionally present a document confirming the applicant’s relationship (or the legality of the representation of the child’s rights) and a document confirming the applicant’s right to stay in the Russian Federation.

3.4. Foreign citizens and stateless persons submit all documents in Russian or together with a duly certified translation into Russian.

3.5. The original passport or other identification document of the parents (legal representatives), and other documents in accordance with paragraph 3.1 of this Procedure are presented to the head of the educational Institution or an official authorized by him within the time frame determined by the founder of the educational Institution, before the child begins visiting the educational Institution.

3.6. An application for admission to an educational institution and the documents attached to it, submitted by the parents (legal representatives) of children, are registered by the head of the educational institution or an authorized official responsible for accepting documents in the journal for accepting applications for admission to an educational institution. After registering the application, the parents (legal representatives) of the children are given a receipt for receipt of documents containing information about the registration number of the application for admission of the child to the educational institution and the list of submitted documents. The receipt is certified by the signature of the official of the educational institution responsible for receiving documents, and the seal of the educational institution (Appendix No. 2).

3.7. Children whose parents (legal representatives) have not submitted the documents necessary for admission in accordance with this Procedure remain on the register of children in need of a place in an educational institution. A place in an educational institution is provided to a child when places become available in the appropriate age group during the year.

3.8. After receiving the documents specified in paragraph 3.1 of these Regulations, the educational institution enters into an agreement on the formation of educational programs for preschool education (hereinafter referred to as the Agreement) with the parents (legal representatives) of the child. Copies of the documents presented upon admission are stored in the educational institution for the duration of the child’s education.

3.10. When a student is enrolled, an education agreement (hereinafter referred to as the Parent Agreement) is concluded between the Institution and parents (legal representatives). The parental agreement includes the mutual rights, obligations and responsibilities of the parties arising in the process of education, training, development, supervision, care and rehabilitation of the student, the duration of his stay in the Institution, as well as the calculation of the amount of fees charged to parents (legal representatives) for maintenance studying at the Institution.

3.11. Admission of a student to the Institution is formalized by order within three working days after the conclusion of the contract. The order is posted within three days after publication on the information stand of the preschool educational institution and on the official website of the preschool educational institution on the Internet.

3.12 When registering a child, parents (legal representatives) give their consent to the processing of the following personal data: last name, first name, patronymic; year of birth, month of birth, date of birth; address, health status, passport details, place of work, position, marital status, medical policy, education, profession, date of registration, military registration data, Taxpayer Identification Number, gender, citizenship, type of identification document, series and number of this document, personal identification number bills, children's birth certificates. In the form: documentary, electronic, oral information (by phone).

3.13. Children with disabilities are admitted to study at the Institution if there is an adapted general education program for preschool education only with the consent of their parents (legal representatives) and on the basis of the recommendations of the psychological, medical and pedagogical commission.
